A variety of disorders\u2014whether psychogenic, organic, or a combination of both\u2014can lead to a reduction in erectile capacity. <\/p>\n<p>An erection is a highly complex sequence of brain, nerve, muscle, and vascular activities. Here are several aspects: <\/p>\n<ul>\n<li>During the resting phase, there is low arterial and venous blood flow in the penis.<\/li>\n<li>Filling phase of the erection: The smooth muscle relaxation of the arteries leads to a drastic increase in blood flow in the artery supplying the penis, while the venous outflow remains constant. The increase in penile volume is supported by the filling of the erectile tissue (corpora cavernosa). <\/li>\n<li>Tumescence = increase in penile volume: After reaching submaximal erectile tissue volume, a rise in pressure occurs, which initially compresses the small veins in the penis, thereby reducing venous outflow. As blood pressure continues to rise, there is neither further arterial inflow nor venous outflow, which causes the increase in volume. Conversely, a lack of arterial inflow reduces the necessary pressure buildup or the ability to maintain pressure. Nocturnal erections are preserved in cases of psychogenic impotence. <\/p>\n<p>The sleep cycle begins with a phase of non-REM sleep (NREM sleep), followed by a very short phase of REM sleep (REM sleep). During a normal night&#8217;s rest, a person typically goes through about four to five REM sleep phases. Penile erections are generally considered a concomitant phenomenon of REM-sleep-related physiological changes in healthy men. Therefore, men very often wake up in the morning with a REM-sleep-related erection, also known as nocturnal penile tumescence (NPT). Typically, the sleep cycle begins with an NREM phase, followed by a very short REM phase. Dreams usually occur during REM sleep <a href=\"#quellen\">[2, 3]<\/a>. REM sleep generally begins 90 minutes after falling asleep. The first REM phase typically lasts 10 minutes, with each subsequent REM phase lengthening, and the last one lasting up to an hour. This means that the proportion of REM sleep in a person&#8217;s total sleep time increases toward dawn. During a normal night&#8217;s rest, a person typically goes through about four to five REM phases. REM sleep accounts for about 20\u201325% of total sleep time in adults, or about 90\u2013120 minutes. Some people tend to wake up immediately after a REM phase or experience a phase of very light sleep <a href=\"#quellen\">[2 &#8211; 6]<\/a>.        <\/p>\n<p>There are many theories about the functions of REM sleep, but they are not yet fully understood. It is assumed that REM sleep fulfills a sentinel function, causing brief but periodic awakening after the organism has been prepared for immediate fight or flight <a href=\"#quellen\">[7, 8]<\/a>. That is, REM sleep periodically activates an animal to scan the environment for potential predators. Such a built-in physiological mechanism would presumably provide maximum security against external dangers while minimally disturbing sleep continuity. Apparently, people are more alert when awakened from REM sleep than from NREM sleep <a href=\"#quellen\">[2, 8, 9]<\/a>.<\/p>\n<\/div><\/div><\/div><div class=\"fusion-layout-column fusion_builder_column fusion-builder-column-4 fusion_builder_column_1_2 1_2 fusion-flex-column\" id=\"testanchor\">Nocturnal Penile Tumescence<\/strong><\/p>\n<p>Spontaneous awakening from sleep is significantly associated with the erection cycle <a href=\"https:\/\/2n8life.com\/en\/guide\/\">[5]<\/a>. Sleep varies individually, but most men have four to five full erections during the REM sleep phase, with each erection lasting about 25\u201335 minutes. Erections are a normal part of the REM sleep phase for men of all ages, even infants and children. The connection between REM sleep and erections is the reason why men frequently wake up directly from the REM sleep phase or shortly after the end of a long morning REM phase. Thus, while men do not always wake up at the end of every REM sleep phase, it is very likely that they wake up in the morning with a REM-sleep-related erection <a href=\"#quellen\">[5]<\/a>. Morning erections are scientifically defined as involuntary sleep-related erections (SREs) or nocturnal penile tumescence (NPT) and are a healthy and normal physiological response experienced by most men <a href=\"#quellen\">[5]<\/a>. Erections are considered a normal part of sleep physiology in men. The assessment of NPT is one of the first tests developed to investigate erectile dysfunction (ED). The mechanism of NPT is believed to be based on neurovascular response mechanisms similar to those in erotically induced erections. Therefore, men with documented normal NPT are assumed to have a normal capacity for spontaneous, erotically induced erections. In fact, the primary function of NPT testing is to distinguish psychogenic from organic causes of erectile dysfunction. Men who do not have erections due to psychological problems can still have erections during deep sleep. As a result of a taboo that still exists in society, ED can lead to high levels of suffering and thus to a significant impairment of quality of life <a href=\"#quellen\">(8)<\/a>. Only about 10 percent of affected men seek medical help after an average waiting period of one and a half years <a href=\"#quellen\">(8)<\/a>.  <\/p>\n<p>In a study examining men aged 20 and older, ED is reported by 20% of men, and as high as 77.5% in men over the age of 75 <a href=\"#quellen\">(1)<\/a>. These figures are also confirmed in daily clinical practice by urologists in private practice <a href=\"#quellen\">(2)<\/a>. <\/p>\n<p>A distinction is made between organic and psychogenic ED, whereby organic ED develops gradually and often occurs alongside high blood pressure, elevated blood lipid levels, or diabetes <a href=\"#quellen\">(1)<\/a>.
